5/28/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C informed family that QMHP-C would continue to practice social distancing such as counseling session being within 6 feet while also having the proper PPE while conducting the session. QMHP-C arrived to the home of Ben-Nita to conduct an individual session to continue to focus on her ISP goals. Upon arrival Ben-Nita was in a happy manner exhibited by her positive body language and interaction. 2. Behavior: Ben-Nita presented hostile behaviors at times within the session as she was discussing certain issues within the home that cause her become stressed or leave the home without her mom’s permission and come back when she wants to come back. 1.Intervention: QMHP-C began the session by assessing Ben-Nita’s emotional wellbeing. QMHP-C conducted a therapeutic session which engaged Ben-Nita in dialogue about her progress with decreasing her anger in the school, community and home setting. QMHP-C assisted Ben-Nita in participating in a role paly in which she given general scenarios of situations that have the potential to occur and assisted her with processing the problems and then determining the best way of rectifying the situation with little to no conflict. 2. Intervention: QMHP-C role-played several of the scenarios to provide Ben-Nita with a better understanding of why simple situations turn into major conflicts to include physical combat and what she needs to do to avoid this. QMHP-C engaged Ben-Nita in a discussion on problem-solving skills with issues at home with family members and with peers in the community and school setting. QMHP-C assisted Ben-Nita with developing a list of interventions to use to prevent future conflicts with her mother, family members and peers in the community. 1. Response: Ben-Nita appeared to be receptive to the suggestions provided to her by QMHP-C during the session. Ben-Nita verbalized what she felt has been her progress and admitted to believing her reason for displaying an angry demeanor is because of her concern with people taking advantage of her if she began to reduce her way of responding when angry. Ben-Nita verbalized frustration with not being able to express her feelings without being told she’s being disrespectful even when she is just attempting to communicate positively in an effort to avoid conflict. 2. Response: Ben-Nita made attempts through utilizing skills learned to try new ways to communicate in a more appropriate manner. Ben-Nita verbalized her plan to stay conscious of conducting herself in an appropriate and positive manner during this upcoming week as she utilizes the skills learned while stating to QMHP-C “don’t hold me to it when I don’t do something that I am supposed to because I said that I was going to try”.
